For your convenience we accept cash, personal checks, American Express, Discover, Visa, MasterCard and Care Credit.
You can click the link below or log-on to patientconnect365.com to process your payment, if you have not registered on the patient portal you will need to contact the office to have an invitation to register resent. We also offer CareCredit as an option if you require a payment plan; CareCredit gives you to opportunity to make interest-deferred payments over a twelve-month period. Many times, a simple telephone call will clear any misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We are not responsible for knowing what your insurance coverage may or may not be. That is between you and your insurance company.
We will send you a monthly statement. Most insurance companies will respond within a few weeks. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated, if your balance is not paid within 60 days of your insurance closing you will be charged a 2% finance charge.
